For the current Cole Williams LAME fork, the maximum-quality recommendation is:
lame.exe -V0 -b 192 --vbr-min-strict --bitrate-boost=3 input.wav output.mp3
This combines the V0 quality target with a strict 192 kbps minimum and the strongest cVBRb bitrate boost. These additional controls belong to the Cole Williams fork and are not available in generic/mainline LAME builds.
If you are using a generic LAME build, use:
lame.exe -V0 input.wav output.mp3

Use -V2 when you want a simple general-purpose recommendation:
lame.exe -V2 input.wav output.mp3
V2 is a high-quality variable-bitrate setting that gives LAME freedom to spend more bits on difficult passages and fewer bits where they are less useful. It is a sensible choice when file size still matters and you do not specifically need the maximum-quality setting above.
Use 192 kbps CBR when broad compatibility and predictable playback are the priority:
lame.exe -b 192 input.wav output.mp3
CBR keeps the nominal bitrate fixed and is widely understood by MP3 players and software. Higher CBR bitrates can provide some increase in quality depending on the source material and the listener's hearing, but they also increase file size.
